Some of the pills were also very expensive!
My care manager took the time to listen to my questions, helped me put a plan in place to ensure I was taking all of my medicine correctly and provided me with resources to supplement the cost of the pills. Our monthly membership plan is for individuals seeking additional support, education, or assistance with the plan of care prescribed by their community physicians.
The TribeHealth program may be fully covered by your insurance carrier. When you complete your intake form, we will be able to confirm any monthly coinsurance or copay amounts.
Our care team does not replace your relationship with your current doctor. We provide enhanced coordination and support to ensure all of your needs are met. Our care managers and physicians communicate with your doctors directly to update them on your clinical status and care needs in between office visits.
